Job Summary

As a RN Referral Liaison Post Acute Hospice, you will play a vital role in creating a positive patient care experience by facilitating problem-solving and decision-making. You will ensure accuracy and timeliness on all functions related to insurance prior authorization, plan benefits, and medical necessity documentation. Your expertise will serve as a liaison between patients, physicians, and the healthcare team.

Key Responsibilities
  • Develop a positive patient care experience by greeting, preparing, exiting, and following up with patients for providers according to established competency guidelines, policies, and procedures.
  • Contribute to a collaborative team environment by participating in opportunities to improve employee and customer satisfaction, including self-development, continuing education, and achieving department objectives and goals.
  • Provide support and collaborate with physicians and multi-disciplinary care teams to ensure best care and results. Ensure accuracy of data collection and reporting for quality initiatives.
  • Maintain accountability for maximizing revenue capture and growth while optimizing cost savings. Communicate opportunities for organizational growth and improvement.
  • Apply evidence-based practice and scientific knowledge in healthcare to nursing practice, ensuring that nursing care is delivered based on patient-specific needs and clinical needs.
  • As an SSM Health nurse, you will demonstrate professional nursing standards defined in the professional practice model. Use the ANA Code of Ethics for Nurses to guide your response to the current and evolving healthcare and nursing needs of our patients and patient populations.
  • Work in a constant state of alertness and safe manner.
  • Perform other duties as assigned.
Requirements
  • Graduate of an accredited school of nursing or education equivalency for licensing.
Work Environment

This role is part of the Intake/Admissions department and requires a 40-hour workweek.
