Licensed Practical Nurse

The role of a Licensed Practical Nurse (LPN) at Avamere Rehab of Rogue Valley is to provide high-quality patient care and support the interdisciplinary team in delivering exceptional services. Key Responsibilities:
- Complete accurate and detailed records for resident care, ensuring compliance with regulatory requirements.
- Coordinate daily services, programs, and activities with nursing staff and support personnel to maintain the resident's total regimen of care.
- Administer medications according to prescribed orders and review medication cards for completeness and accuracy.
- Review and update resident care plans regularly to reflect changes in their needs and goals.
- Lead and guide nursing personnel, ensuring direct care is provided in accordance with resident care plans and wishes.
- Maintain effective communication with residents, family members, and other stakeholders involved in resident care.
- Participate in facility surveys by authorized government agencies.
Requirements:
- A current, active license to practice as an LPN in this state.
- Degree from an accredited college or university.
- Knowledge of nursing practices, procedures, laws, regulations, and guidelines related to long-term care facilities.
- Ability to document patient care accurately and confidentially, adhering to HIPAA guidelines.
