Care Coordinator

We are seeking a highly motivated and compassionate Care Coordinator to join our team at The Burton Center For Disabilities & Special Needs. As a key member of our staff, you will play a vital role in implementing an effective vocational training program for adults with disabilities and special needs.
The ideal candidate will have a valid SC Driver's license, a high school diploma or equivalent, and at least one year of experience working with individuals with disabilities and special needs.
The estimated annual salary for this position is $35,000-$45,000 based on location and experience.
Key Responsibilities:- Assist individuals with self-help skills (i.e., toileting, eating, etc.)
- Implement facilitated plans to promote independence and personal growth
- Monitor individuals and complete monthly progress reports on all objectives
- Collaborate with workshop supervisors to evaluate and place individuals in various jobs
- Transport individuals to and from Burton Center and on field trips while maintaining accurate records
