We are seeking a highly skilled and experienced Telehealth Clinical Pharmacist to join our eICU team at Sutter Health. As a critical care pharmacist, you will play a vital role in providing patient-centered clinical services, medication information, and education to patients remotely across multiple Sutter Health hospitals.
Key Responsibilities- Deliver pharmaceutical care through the provision of patient-centered clinical services, medication information, education, and medication preparation and distribution.
- Exercise sound judgment and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner.
- Gain confidence and cooperation from patients, their families/support groups, and other healthcare providers through competent patient assessment, attentive monitoring, and care, and effective communication.
- Adhere to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering optimal patient care.
- May also be responsible for performing specific tasks and/or orienting other staff to the department.
- Doctorate: Graduation from an accredited school of Pharmacy.
- CERTIFICATION & LICENSURE: PHARMD-Licensed Pharmacist or PHARMR-Registered Pharmacist.
- ACLS-Advanced Cardiac Life Support (for those who will be responding to codes).
- TYPICAL EXPERIENCE: 1 year of recent relevant experience.
This position may regularly work, store, prepare, receive, unpack, transport, dispose of, or administer drug(s) identified as hazardous, or potentially hazardous, by the National Institute for Occupational Safety and Health (NIOSH) for purposes of USP 800.
