About the Role

We are seeking a highly skilled and compassionate Outreach Coordinator and Consultant to join our team at RHA Health Services. This is a unique opportunity to make a meaningful impact in the lives of individuals with intellectual and developmental disabilities, mental health, and substance use challenges.

Key Responsibilities
  • Provide case management services to the Deaf and Hard of Hearing population on a regional level, relying on experience, knowledge, and education to deliver high-quality services.
  • Plan and facilitate consumer transitions to other services and local agencies in an efficient manner.
  • Maintain regular contact with individuals served, their treatment teams, guardians, and significant others.
  • Assure consistent provision of quality services, assessing individuals' needs and providing linkages/referrals to relevant resources.
  • Represent RHA at local management entity and other necessary meetings related to the care of individuals served.
  • Perform other duties as assigned.
Requirements
  • Master's Degree in a related field and one year of supervised, post-graduate experience serving individuals with mental health or substance abuse, or a Bachelor's Degree in a Human Service field with two years of supervised, post-graduate experience serving individuals with mental health or substance abuse.
  • Must have knowledge and experience with the age and population served.
  • Be Fluent in sign language, as determined by a Sign Language Proficiency Interview (SLPI) score of Advanced Plus or better.
