POSITION SUMMARY:

The Practical Nursing Specialist plays a crucial role within the Interdisciplinary Care Team (IDT), contributing to the evaluation, planning, and execution of nursing services.

This role involves administering medications, treatments, and enteral feedings; observing each client's physical and psychological condition; and facilitating necessary medical follow-ups.

The Practical Nursing Specialist educates the IDT on each client's unique health care requirements and fosters client autonomy.

This position operates within the framework of Pennsylvania's Nurse Practice Act and adheres to Elwyn's policies and procedures as well as those set forth by regulatory agencies.

DUTIES & RESPONSIBILITIES
- Conduct a thorough nursing assessment upon admission/readmission and as health needs evolve.
- Perform a Quarterly Nursing Assessment every three (3) months, encompassing a review of systems, comprehensive physical examination, skin evaluations, Braden Scale, and Fall Risk Assessment.
- Complete a Discharge Summary upon client discharge, detailing all relevant medications, medical follow-ups, and essential health care information.
- Present clinical data gathered through direct observation, interviews, physical assessments, and other reports to the IDT in a clear and organized manner.
- Prioritize nursing tasks based on professional judgment.
- Develop a health care plan for clients requiring continuous nursing care, specifying nursing goals and strategies to meet the client's health care needs.
- Review and update the health care plan at least every 90 days, incorporating data from nursing assessments and communication with the IDT.
- Collaborate with Day and Work Services to ensure that clients' health care needs are communicated effectively and necessary documentation is forwarded.
- Refer clients to physicians when assessments indicate further medical evaluation is necessary; facilitate transfers to hospitals or Emergency Departments as required.
- Complete transfer documentation for hospitals and Emergency Departments, ensuring all pertinent information is forwarded to the receiving facility.
- Monitor weight and blood pressure at least monthly, or more frequently as indicated by nursing judgment or physician orders.
- Prepare and administer medications as prescribed by the physician, following Elwyn's medication policies and procedures; obtain verbal orders when necessary and document as per policy.
- Administer and record prescribed medications and treatments in both hard copy and electronic formats, ensuring client privacy during administration.
- Provide enteral tube feedings, blood glucose testing, nebulizer treatments, and skin care as ordered by the physician, maintaining client privacy.
- Instruct direct care staff on the administration and documentation of specific treatments, including topical creams and over-the-counter agents.
- Report any observed side effects of medications and treatments, as well as relevant laboratory values, to the Health Care Coordinator/Charge Nurse promptly; contact the physician as directed.
- Review the monthly Medication Administration Records (MARs) for accuracy and update as necessary; remove outdated MARs and replace them with current ones at the beginning of each month.
- Assess client progress in self-administering medications and promote increased independence in client functioning.
- Conduct monthly medication exchanges with the pharmacy provider and address any identified issues immediately.
- Administer and document immunizations as prescribed, ensuring necessary consents are completed prior to administration.
- Prepare clients for medical appointments by communicating necessary instructions and maintaining NPO (nothing by mouth) status, procedural preparations, and administering pre-sedation medications.
- Ensure all scheduled lab work is completed and documented; if not, arrange follow-up and document accordingly.
- Document all behavioral and physical changes in the client's status and other relevant information in nursing progress notes.
- Communicate with family members/guardians regarding any new injuries requiring more than basic First Aid or as requested.
- Relay important health issues to staff through the program's Alert Log.
- Document all relevant information in the MAR, nursing progress notes, and 24 Hour Change in Status Report.
- Communicate all pertinent client and unit information to incoming nurses through written reports.
- Provide significant clinical updates to the Charge Nurse throughout the shift and contact the Charge Nurse by phone one hour prior to the end of the shift for a final verbal report.

- Inspect living areas for potential hazards and report findings to the Unit Director, Health Care Coordinator, or Charge Nurse.

- Monitor for potential infectious diseases among clients and staff, reporting to the Health Care Coordinator/Charge Nurse, who will notify the Director of Nursing.

- Provide clear guidance to Elwyn staff during medical emergencies until the Charge Nurse arrives.

- Order and restock medical and nursing supplies for the assigned unit as needed.
- Maintain medical equipment in proper working order in all medication dispensaries; clean equipment monthly and facilitate repairs as necessary.
- Keep medication dispensaries clean and organized at all times; sanitize medication carts, countertops, and work surfaces at the end of each shift.
- Assist in training and orienting new LPN employees.
- Provide nursing coverage across all units as needed, demonstrating flexibility and teamwork when nursing assignments change.

- Ensure timely reporting of all injuries or occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals, in accordance with facility policies and OSHA guidelines.

- Collaborate with Risk Management as necessary.
- Adhere to Elwyn's policies and procedures regarding HIPAA and the protection of health information.
- Complete all mandatory educational requirements by the annual due date.
- Obtain a minimum of 24 hours of in-service training annually; attend nursing meetings and in-services as required.
- Perform cardiopulmonary resuscitation (CPR) and crisis intervention using agency-trained protocols.
- Submit current Nursing License, CPR card, Driver's License, physical exam, and contact sheet by the specified due dates.
- Perform additional duties as assigned.
EDUCATION/EXPERIENCE/

SKILLS REQUIREMENTS:
- Licensed as a Practical Nurse in Pennsylvania.
- Minimum one (1) year of general nursing or medical-surgical experience required; experience in Developmental Disabilities nursing preferred.
- Current CPR certification.
- Proficient in using electronic medical record systems.
- Strong planning, communication, customer interaction, collaboration, and documentation skills required.
- Ability to work effectively as part of a team.
- Capable of thriving in a fast-paced environment while managing and prioritizing multiple, often competing, tasks.
- Strong attention to detail.
- Excellent time management and organizational skills.
- Demonstrated judgment with the ability to independently solve problems and make decisions with minimal supervision.
- Ability to anticipate future changes or identify problem areas and take effective actions.
- Must be able to perform CPR and crisis intervention techniques using agency-trained protocols.
- Basic experience with Microsoft Office applications, including Word and Outlook.
- Must possess a current, valid driver's license in the state of residence, with three years of driving experience in the United States and an acceptable driving record.
