Primary Care Physician Leader

ChenMed is seeking a highly skilled and experienced Lead, Primary Care Physician to join our team. As a Lead, Primary Care Physician, you will be responsible for providing high-quality patient care, leading a team of healthcare professionals, and contributing to the growth and success of our organization.
Key Responsibilities:- Provide exceptional patient care, including assessment, diagnosis, treatment, and management of acute and chronic illnesses.
- Lead a team of healthcare professionals, including care promoters, care facilitators, and care coordinators, to ensure seamless patient care.
- Develop and implement effective care plans, taking into account patient needs, preferences, and goals.
- Collaborate with other healthcare professionals, including specialists and hospitalists, to ensure comprehensive patient care.
- Monitor patient outcomes, identify areas for improvement, and implement changes to optimize patient care.
- Participate in quality improvement initiatives, including data analysis, process improvement, and education.
- Develop and maintain strong relationships with patients, families, and caregivers to ensure patient-centered care.
- Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
- Current, active MD licensure in the State of employment.
- A minimum of 1-year clinical experience in geriatric, adult, or family practice setting.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross.
- Ability to work independently and collaboratively as part of a team.
