Serves as a vital member of the hemodialysis healthcare team as a Registered Nurse Applicant, under the guidance of a Staff Registered Nurse, to ensure the delivery of high-quality patient care daily in alignment with FMS policies, procedures, and training. Upholds the mission, vision, values, and customer service philosophy of FMCNA. Engages in the Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) activities, focusing on patient satisfaction and actively participating in initiatives that promote the achievement of FMCNA Quality Enhancement Goals (QEP). Complies with all FMCNA Compliance Program requirements and FMS patient care and administrative policies. Employees in this role must secure their nursing license and registration within the timeframe specified by state regulations or within 90 days from the hire date, whichever is sooner.
KEY DUTIES AND RESPONSIBILITIES:
STAFF RELATED:
- Oversees Patient Care Technicians in delivering safe and effective chronic hemodialysis therapy to patients, adhering to standards outlined in facility policy manuals and regulations set by corporate, state, and federal agencies.
- Assigns tasks to all direct patient care staff, including LVN/LPNs, Patient Care Technicians, and Dialysis Assistants.
- Evaluates daily patient care requirements and formulates suitable patient care assignments.
- Regularly monitors patient care staff for compliance with techniques and adherence to facility policies and procedures.
- Participates in staff training and onboarding of new personnel as assigned.
- Attends all mandatory staff meetings as scheduled.
Education:
- Ensures that the educational needs of patients and their families regarding End Stage Renal Disease (ESRD) are met.
- Provides continuous education to patients about their renal condition, vascular access, dialysis therapy, and other related health issues.
- Discusses and documents education concerning diet/fluid and medication adherence with patients.
- Offers patient-specific education regarding adequacy measures where applicable - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM).
- Promotes awareness of transplant options, modality choices, and reduction of catheter use.
- Educates patients on laboratory values and their relationship to effective dialysis therapy, treatment compliance, medications, and fluid management.
- Delivers safe and effective care to patients with ESRD.
- Accurately implements treatment prescriptions, including Sodium (Na) modeling and Ultrafiltration modeling, to ensure stable treatment therapy as indicated.
- Assesses patients' responses to hemodialysis treatment, making necessary adjustments to the treatment plan as directed by the prescribing physician. Communicates any issues or concerns to the Team Leader or physician.
- Identifies and reports patient-related issues to the Team Leader or physician.
- Initiates Initial and Annual Nursing Assessment, along with ongoing evaluation and documentation of patient care needs according to FMC Policies and Procedures.
- Actively participates in the pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of patients receiving hemodialysis treatment therapy in accordance with established FMC procedures.
- Takes appropriate actions for changes in patient adequacy status and troubleshooting access flow issues as indicated by OLC/AMP alerts.
- Provides, supervises (if applicable), and monitors hemodialysis access care following established procedures.
- Implements, administers, monitors, and documents patients' responses to prescribed interdialytic transfusions, including notifying the physician and appropriate blood supplier of any adverse reactions.
- Ensures accurate and complete documentation by Patient Care Technicians on the Hemodialysis Treatment Sheet.
- Reviews, transcribes, and accurately enters physician lab orders into the Medical Information System.
- Ensures proper preparation of lab requisitions for Spectra or alternate labs.
- Ensures correct lab tubes are utilized for prescribed lab specimens and that lab draw and processing procedures are followed for all samples.
- Identifies and ensures appropriate follow-up regarding missed labs and specimens reported as insufficient according to company policies.
- Ensures all specimens are packaged according to Department of Transportation (DOT) policies and procedures for the shipment of blood or body fluid specimens and potentially hazardous materials.
- Directs and delivers all labs to the appropriate facilities.
- Reports alert/panic and abnormal lab results to the appropriate physician.
- Ensures lab results are forwarded to physicians as requested.
- Enforces all company-approved policies and procedures, along with regulations set by state and federal agencies.
- Maintains overall shift operations in a safe, efficient, and effective manner.
- Acts as a resource for other staff members.
- Regularly meets with the Clinical Manager to discuss personnel and patient care status, issues, and information.
- Collaborates and communicates with physicians and other healthcare team members to interpret, adjust, and coordinate patient care.
- Provides assistance to patients regarding prescription refills according to FMCNA Policies.
- Ensures timely transcription and entry of all physician orders into the Medical Information System.
- Oversees all documentation of patient information.
- Maintains the facility drug list for all required stock medications.
- Maintains competency with all emergency operational procedures and initiates CPR and emergency measures in case of cardiac and/or respiratory arrest.
- Ensures verification and availability of adequate emergency equipment.
- Provides appropriate vaccinations, immunizations, and annual Tuberculosis (TB) testing.
- Administers medications as prescribed or in accordance with approved algorithms, documenting appropriate medical justification if indicated.
- Administers PRN medications as prescribed and completes documentation of effectiveness assessment.
- Maintains accurate recording of controlled substances as required by law.
- Assists with coordinating patient transportation if necessary.
- Ensures a clean, safe, and sanitary environment in the dialysis facility treatment area.
- Ensures competency in the safe and effective operation of all dialysis-related equipment.
- Ensures that all patient stations, including machines and chairs, are clean and free of blood and properly positioned.
- Ensures immediate addressing of all blood spills according to FMCNA Bloodborne Pathogen Control Policies.
General
- Ensures all relevant data, including physician orders, lab results, vital signs, treatment parameters, and patient status, are documented and entered into the Medical Information System.
- Ensures all patient-related treatment data is accurately entered into the Medical Information System.
- Ensures compliance with FMCNA policies regarding patient admission, transfer, and discharge.
- Verifies the accuracy of Patient Care Technician documentation.
- Reviews and ensures daily completion of Hemodialysis Treatment Sheets by all patient care staff.
- Ensures adherence to all procedures regarding opening and closing, including monitoring that all staff and patients have safely exited the premises.
- Initiates, documents, and completes ongoing Continuous Quality Improvement (CQI) activities, including monthly reports.
- Completes monthly nursing progress notes.
- Ensures patient medical records are complete with appropriate information, documentation, and identification on each page (Addressograph label on all chart forms).
- Reviews transplant status and follows established procedures for appropriate actions.
- Completes patient care plans for new patients within the initial 30 days or for any patients deemed unstable requiring monthly care plans.
- Completes any long-term programs that are due. Annually
- Completes initial and annual Nursing History and Assessment physical.
- Ensures completion of Annual Standing Order Review with each physician as required.

The employee may occasionally be required to move, with assistance, machines and equipment weighing up to 200 lbs., and may lift chemical and water solutions weighing up to 30 lbs. as high as 5 feet.
