About the Role

We are seeking a highly skilled and compassionate Primary Care Nurse Practitioner or Physician Assistant to join our team at NC Elder Care. As a key member of our healthcare team, you will provide high-quality primary care to patients in various long-term care settings, collaborating with facility staff and primary care/specialty providers to deliver exceptional care.

Responsibilities
  • Provide primary care to patients in assigned long-term care facilities, including medical evaluations, diagnoses, and treatment plans.
  • Work collaboratively with primary care/specialty providers to ensure comprehensive and coordinated care.
  • Develop and maintain strong relationships with facility staff, families, and community providers to foster a culture of care and support.
  • Document patient care and treatment plans accurately and thoroughly, using electronic health records (EHRs) and other documentation tools.
  • Participate in on-call/after-hours call rotations, as needed, to ensure 24/7 patient care.
Qualifications and Requirements
  • Certified Nurse Practitioner (ANP, GNP, FNP, AGNP) through a national board (ANCC, AANP).
  • Master's degree in Nursing or a related field.
  • Current, unrestricted license to practice as a Registered Nurse (RN) and Nurse Practitioner (NP) in North Carolina or the ability to obtain.
  • Unrestricted DEA license to prescribe narcotics, per state regulations.
  • Access to reliable transportation for travel to client and patient sites within a designated area.
Preferred Qualifications
  • Experience working in long-term care settings, including skilled nursing facilities (SNFs), assisted living facilities (ALFs), group homes, and continuing care retirement communities (CCRCs).
  • Proficiency with computer systems and electronic health records (EHRs).
  • Understanding of geriatrics, chronic illness, and advanced/terminal illnesses.
Benefits and Schedule
  • Full-time, contract position with a competitive salary range of $95,000.00 per year.
