Home Health Registered Nurse

We are seeking a skilled and compassionate Home Health Registered Nurse (RN) to provide high-quality nursing care to patients in the comfort of their homes. As a Home Health RN, you will be responsible for assessing patients' health needs, developing individualized care plans, administering medications, and educating patients and their families on managing their conditions.
Responsibilities- Conduct thorough assessments of patients' health status and develop individualized care plans in consultation with the healthcare team.
- Administer medications, treatments, and wound care as prescribed by the physician.
- Monitor patients' vital signs and symptoms and report any changes or concerns to the physician promptly.
- Educate patients and their families on proper healthcare strategies, disease prevention, and medication management.
- Collaborate with other healthcare professionals, such as LVNs and therapists, to provide holistic care to patients.
- Coordinate and communicate effectively with the interdisciplinary team, physicians, and other healthcare providers.
- Document all patient care activities accurately and timely in electronic health records.
- Provide emotional support and encouragement to patients and their families.
- Ensure compliance with all applicable laws, regulations, and agency policies, such as OSHA and CDC guidelines for Universal Precautions in home health care.
- Graduate of an accredited school of nursing with a valid and current Registered Nurse (RN) license by the Texas State Board of Nurse Examiners.
- Preferred minimum of 1 year of clinical nursing experience, preferably in home health or a related field with a minimum of 2 hours of general, critical care, or emergency hospital experience.
- Strong clinical assessment and critical thinking skills.
- Skills and knowledge with infusion therapy and wound care are a bonus but not required.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Proficient in using electronic health records and other healthcare software.
- Compassionate, empathetic, and patient-centered approach to care.
- Reliable transportation and valid driver's license.
- Basic Life Support (BLS) certification.
