We are seeking a compassionate and skilled Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing personalized care and support to our patients in the comfort of their own homes.
Key Responsibilities- Provide direct patient care under the direction of a registered nurse and according to the patient's plan of care.
- Assist patients with self-administered medications, including opening bottle caps, reading labels, and observing medication intake.
- Take accurate temperature, pulse, and blood pressure measurements, and report any abnormal results to the supervisor.
- Help patients maintain good personal hygiene, including bathing, grooming, skin care, and other activities.
- Assist with feeding patients, including communicating basic nutrition principles and observing food and fluid intake.
- Provide necessary skills to safely assist patients with mobility, exercises, positioning, transfers, and ambulation.
- Document interactions with patients, caregivers, doctors, and other staff members, and report changes and document pertinent information and care rendered to patients.
- High school diploma or equivalent.
- Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
- Must meet applicable state certification requirements.
- At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
