About the Role

We are looking for a dedicated and experienced Private Duty Nurse LPN to join our team. As a Private Duty Nurse LPN, you will be responsible for providing one-on-one care to our patients in their homes, including administering medications, wound care, and other medical treatments as prescribed by our patients' physicians. You will also be responsible for documenting patient care and communicating with our patients' families and healthcare teams.

Responsibilities
  • Implement physician-prescribed plans of care for our patients
  • Conduct patient assessments and coordinate care with our healthcare teams
  • Administer prescribed medications, treatments, and therapies to our patients
  • Document patient care and communicate with our patients' families and healthcare teams
  • Provide education and training to our patients and their families on disease management and self-care
Requirements
  • Active, unencumbered LPN license in the state where you will be practicing
  • Current CPR certification from the American Red Cross or American Heart Association
  • TB skin test (current within the last 12 months)
  • At least six months of prior hands-on nursing experience
