Job Summary: We're seeking a skilled and compassionate Hospice LPN to join our team. As a Hospice LPN, you'll provide nursing interventions and teaching skills to maximize patient comfort and enhance the quality of life for patients and their families.
Estimated Salary: $65,000 - $85,000 per year, depending on experience and location.
Job Responsibilities:
* Under the direction of an RN, provide nursing tasks to patients and families, such as continuous care, dressing changes, and catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG).
* Demonstrate clinical skills regarding application of Company policies and procedures, including medication administration, symptom management, and patient/family relationships.
* Observe and report signs of adverse reactions and responses to treatments to the RN.
* Perform venipuncture for blood specimens and document patient information per policy and procedure in the medical record.
* Participate in Quality Assessment and Performance Improvement activities as appropriate for the position.
Required Skills and Qualifications:
* A high school diploma or G.E.D. or an equivalent combination of education and experience.
* Current LPN licensure in the state of employment.
* IV certification required at time of hire beginning November 10, 2024. Employees in this job prior to November 10, 2024 will be required to obtain certification by May 10, 2025.
* Active BLS for healthcare professionals from the American Heart Association or Red Cross.
* Valid driver's license and automobile insurance per Company policy.
* Reliable transportation to meet visit schedule.
