The Clinical Nurse Case Manager is responsible for advocating for patient-centered care by orchestrating the care plan during the patient's stay, managing the duration of the stay, ensuring effective resource management, and developing a safe discharge plan in collaboration with the multidisciplinary team.

This role involves:

  • Facilitating care transitions and progression using established criteria.
  • Coordinating activities that enhance quality outcomes and patient throughput while balancing optimal care with resource utilization.
  • Documenting findings from assessments and communicating the needs and preferences of patients and their families.

Qualifications include:

  • Associate Degree in Nursing or Nursing Diploma required.
  • Bachelor's Degree in Nursing preferred.
  • Minimum of 2 years of experience in case management or 3 years in clinical nursing required.

Key Responsibilities:

  • Identify patients at risk for adverse outcomes during care transitions.
  • Conduct comprehensive assessments of patients' psychosocial, medical, and discharge needs.
  • Develop strategies to mitigate readmission risks through education and resource access.
  • Collaborate with the healthcare team to coordinate care plans and ensure successful transitions.
  • Document care coordination interventions and maintain effective communication with all stakeholders.
