Nurse Specialist for Community Support

The City of Chesapeake seeks a skilled Licensed Practical Nurse to join our team in providing high-quality care and support to individuals with intellectual and physical disabilities.
Key Responsibilities:
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Provide technical support to non-licensed staff on health care services and medication administration.
- Engage in active treatment with residents, including dispensing medications and performing nursing tasks as directed by physicians.
- Maintain accurate records and inventory of medications, ensuring compliance with federal, state, and local regulations.
- Develop and update nursing care plans, participate in quarterly and annual reviews, and collaborate with Qualified Intellectual Disability Professionals (QIDPs) on changes to individualized care objectives.
- Provide oversight to ensure the health and safety of all individuals in the environment, identifying and defusing crisis situations.
- Assist residents during mealtimes and share meals with residents unless pre-authorized by the supervisor for a separate meal period.
- Perform other duties as assigned.
Requirements:
- Diploma in practical nursing.
- Minimum of one year of related, full-time equivalent experience.
- Licensure as a Licensed Practical Nurse by the Commonwealth of Virginia or a reciprocal state.
- Certification in CPR, Multi-media First Aid, and Handle With Care.
- Valid driver's license and a driving record that is in compliance with the City's policy.
Preferred Qualifications:
- Experience working with individuals with intellectual disabilities in a group home setting.
