About the Role

We are seeking a skilled RN Care Coordinator to join our Elder at Home care team. As a key member of our team, you will provide customized care coordination services and management of transitions of care for our most medically fragile patients and their families/caregivers.

Key Responsibilities
  • Provide across-the-continuum care coordination services to patients and their families/caregivers
  • Manage transitions of care to ensure seamless patient care
  • Develop and implement patient-centered interventions and engagement strategies to promote self-management in the home/community setting
  • Collaborate with our provider group of eight physicians to provide urgent visits and assessments in the home
Requirements
  • Oregon Registered Nurse License upon hire
  • 2 years of clinical medical-surgical, acute care, or long-term care experience, including 1 year in community-based/ambulatory care and 1 year in care of the frail elderly
Preferred Qualifications
  • Bachelor's Degree in Nursing
  • Home health, hospice, palliative care, managed care, or skilled nursing facility background
