InnovAge, a leading provider of healthcare and social support services, is seeking a skilled Dietitian to join their team. As a Dietitian, you will play a critical role in providing nutrition assessments, planning, and education to participants, caregivers, and staff.

Key Responsibilities:
  • Nutrition Planning and Education:
    • Develop and implement personalized nutrition plans for participants, taking into account their dietary needs and preferences.
    • Provide education and training to caregivers and staff on nutrition-related topics, including meal planning and preparation.
    • Collaborate with the interdisciplinary team to ensure comprehensive care and support for participants.
  • Participant Care:
    • Conduct regular nutrition assessments and reassessments to monitor participants' progress and adjust plans as needed.
    • Develop and implement nutrition care plans for high-risk participants, including medical therapy recommendations.
    • Communicate with In-Home Service providers regarding participants' nutritional needs and preferences.
  • Administrative Responsibilities:
    • Conduct sanitation and safety checks for kitchen and kitchen equipment.
    • Monitor the adequacy of lunch and snack menus and make recommendations for improvement.
    • Update the sack lunch list, supplement sheet, diet lists, and assisted dining room guide as needed.
    Requirements:
    • Bachelor's degree in food science, nutrition, and/or dietetics.
    • Current medical terminology and the ability to complete written assessments and document participant care.
    State Requirements:
    • CA: Certification by CDR as a registered dietitian.
    • CO: Registration with CDR as Dietitian.
    • FL: Registration with CDR as Dietitian; StateServ Certification obtained within 30 days of employment.
    • NM: Current American Dietetic Association registration.
    Preferred Qualifications:
    • Bilingual.
