Hospice Case Manager RN

The registered nurse plays a vital role in planning, organizing, and directing home care services. With a focus on community health education and experience, this professional nurse builds on the resources of the community to develop and implement individualized plans of care for patients and families within their homes and communities.
Key Responsibilities- Develops and initiates the plan of care, working in partnership with the patient, representative (if any), and caregiver(s) to ensure ongoing interdisciplinary assessment and development of the individualized plan of care.
- Initiates proper preventative and rehabilitative nursing procedures, providing services ordered by the physician as indicated in the plan of care.
- Provides patient, caregiver, and family counseling, as well as patient and caregiver education.
- Prepares clinical notes and communicates with the physician responsible for the home health plan of care, as well as other healthcare practitioners as appropriate.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Must have graduated from an approved school of nursing and be licensed in the state where practicing.
- Minimum of two years of experience, with at least one year in public health or home care nursing preferred.
- Maintains a current CPR certification and is a licensed driver with an automobile that is insured in accordance with state or organization requirements.
- Self-directed and able to work with minimal supervision, demonstrating excellent observation, problem-solving, verbal and written communication skills, and nursing skills per competency checklist.
