Job Title: Habilitation Specialist

WellLife Network is seeking a skilled Habilitation Specialist to join our team. As a Habilitation Specialist, you will play a vital role in designing and implementing individual program plans, daily activity schedules, and community integration for our participants.

Key Responsibilities:
  • Lead instructor for assigned unit, responsible for ensuring that all unit activities enhance and support each person's social responsibility, independence, dignity, and continued growth.
  • Participate in all Interdisciplinary Team Conferences for all assigned participants, acting as a liaison between the day habilitation program and residence.
  • Complete all documentation and required paperwork, including Annual and Semi-Annual day habilitation and staff actions plans, within specified time frames.
  • Responsible for planning, writing, and implementing goals designed to maximize each participant's level of independence, as determined by the IDT.
  • Monitor participant's progress by collecting, documenting, and analyzing performance data.
  • Establish and implement appropriate activity schedules for each individual.
  • Communicate with residences and/or families regarding progress, incidents, etc.
  • Serve as a positive role model for participants and staff members in a variety of program and community settings.
  • Provide behavioral interventions when necessary, following the least restrictive OPWDD approved techniques required by the situation.
  • Ensure that the unit is clean, safe, stimulating, and appropriately decorated at all times.
  • Participate and attend meetings/training at various sites.
  • Participate in all routine program functions such as fire drills, mealtimes, arrivals, and departures.
Requirements:
  • Associates degree required in Education, Human Services, or related field.
  • Valid NYS driver's license and two years' experience working with people with developmental disabilities.
