Nurse Practitioner in Family Medicine

The Nurse Practitioner specializing in Family Medicine will contribute to our commitment to providing accessible, high-quality, and patient-focused healthcare to the community. This role involves delivering direct patient care and collaborating with various healthcare and educational partners.
Key Responsibilities:
- Assumes comprehensive responsibility for patient care from diagnosis to recovery.
- Conducts thorough physical assessments, including gathering detailed medical histories and ordering diagnostic tests.
- Identifies and manages both chronic and acute health conditions, addressing complications that may arise.
- Develops and documents care plans, ensuring detailed records for review by medical professionals.
- Initiates and manages medication therapies, monitoring for potential reactions.
- Determines patient recovery and discharges individuals when appropriate.
- Engages in Quality Improvement initiatives within the Family Medicine Patient-Centered Medical Home.
- Collaborates effectively with medical staff, fostering an environment of consultation and teamwork.
- Performs other related duties as assigned.
Education and Licensure:
- Must possess a valid Registered Nurse license and Nurse Practitioner certification from the appropriate regulatory body.
- Current certification in Basic Life Support (BLS) for Healthcare Providers is required.
- A valid driver's license or alternative transportation method is necessary for job-related functions.
- A minimum of two years of relevant clinical experience is required.
- Ability to engage effectively with diverse cultural and ethnic groups.
- Strong communication skills, particularly in clinical environments.
- Demonstrated sensitivity towards underserved populations.
- Proficient in developing individualized care plans and prioritizing tasks.
- Physical demands include frequent standing, walking, and occasional stooping.
- Work may involve confined spaces and requires adaptability to various physical conditions.
- Ability to manage multiple tasks with sustained focus and analytical thinking.
- Frequent engagement in complex problem-solving and decision-making processes.
- Salary range is competitive and commensurate with experience.
- This is a full-time position.
