Job Summary

We are seeking a skilled Transplant Coordinator RN to join our team. As a key member of our transplant program, you will play a critical role in ensuring the smooth transition of patients through the healthcare system.


Key Responsibilities
  • Assess patient needs and create personalized care plans to ensure optimal outcomes.
  • Coordinate tests and procedures for transplant candidates, identifying any issues and arranging necessary care.
  • Manage post-transplant care for recipients, including arranging admissions, lab tests, and clinic visits.
  • Collaborate with patients, physicians, and other healthcare providers to ensure seamless care.
  • Teach patients and families about transplant evaluation, organ allocation, waiting, post-operative care, and immunosuppression effects.
  • Use insurance knowledge to assist with post-transplant prescriptions and coverage issues.

Requirements
  • Associate's degree in nursing.
  • Two years of professional experience as a RN.
  • Basic Life Support skills required or obtain BLS certification within 30 days if not already held.
