Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at Eden Health. As a Home Health Nurse, you will provide high-quality patient care and support to individuals in their homes throughout Sacramento, CA.

Key Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for Home Health patients
  • Administer ongoing care and case management, providing necessary follow-up as directed by the Clinical Manager
  • Implement patient care plans in conjunction with patients and families to achieve optimal resolution of needs and problems
  • Coordinate and oversee the work of Home Health Aides and provide written instructions reflecting current plans of care
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing necessary clinical documentation
Requirements
  • Graduate of an accredited nursing program
  • Current California Registered Nurse (RN) license required
  • Minimum of one year of experience as an RN in an acute- or sub-acute setting
  • Home Health experience preferred, but we are willing to train the right candidate
