Job Description

We are seeking a Patient Care Manager Senior to join our team. The successful candidate will report directly to the Executive Director or Administrator and be responsible for assisting branch leadership with maintaining a high-performing hospice program. This includes managing, overseeing, and directing patient care activities to ensure the delivery of quality patient care.

  • Key Responsibilities:
  • Managing and overseeing clinical and administrative functions, including documentation and record-keeping.
  • Directing the work of clinical associates, including scheduling visits and ensuring timely completion.
  • Preparing for and facilitating the Interdisciplinary Group (IDG), as assigned.
Required Skills and Qualifications

To be successful in this role, you will need:

  • A graduate degree in nursing, preferably a bachelor's degree.
  • Current Registered Nurse licensure in the state of residence and requested employment.
  • At least three years' experience of direct patient care and two years' experience in hospice or home-based care settings.
  • Demonstrated knowledge and compliance with accepted hospice principles and practice.
