About the Role:

The Healthcare Support Specialist will work collaboratively with our interdisciplinary team to provide direct assistance with activities of personal care, maintenance of patient health, and facilitation of treatment plans. Key responsibilities include:

  • Providing daily living support under the supervision of a Registered Nurse or Therapist
  • Reviewing and implementing care plans in collaboration with the Case Manager
  • Notifying the Case Manager of any concerns or issues related to care plan implementation
  • Exhibiting excellent communication and interpersonal skills when interacting with patients, family members, and healthcare professionals
  • Maintaining accurate records and completing mobile care reports upon arrival and departure from patient homes

Requirements:

To be considered for this role, applicants must possess:

  • A high school diploma or equivalent
  • Previous experience in healthcare or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
