Description

Brighton Hospice is a rapidly growing industry leader seeking positive, compassionate, and knowledgeable individuals to join our team.

We recognize that the dying process is a natural part of living and focus on enhancing the quality of remaining life.

Our On-Call team members are a vital part of our care team, providing initial and ongoing assessment of the impact of the terminal diagnosis on daily living and the patient's physical, functional, spiritual, psycho-social, and environmental needs.

This role requires a non-traditional schedule, with a common on-call schedule being 5:00 p.m. to 8:00 a.m. with 7 days on and 7 days off.

This is a salaried role that involves partnering with an interdisciplinary team to implement and manage plans of care, determining scope, priority, and frequency of services based on patient/family needs, and recommending revisions as necessary.

Key Responsibilities:

  • Provide initial and ongoing assessment of the impact of the terminal diagnosis on daily living and the patient's physical, functional, spiritual, psycho-social, and environmental needs
  • Partner with an interdisciplinary team to implement and manage plans of care
  • Consult with and educate the patient/family regarding the disease process, self-care techniques, end-of-life care, and ethical concerns
  • Communicate with the medical director and other clinicians regarding changes in patient needs and outcomes of intervention
  • Prepare and submit documentation of observations, findings, and services provided

Qualifications
