Job Description:

We are looking for a compassionate and experienced Wound, Ostomy and Continence Nurse (WOCN) to join our team. As a WOCN, you will use your clinical skills and expertise to deliver 1:1 skilled nursing, intervention, and supportive care to patients in their homes.

  • Conduct wound care consultations and provide oversight to ensure patients receive the best possible care.
  • Perform client assessments, provide education to patients and/or their families, and monitor patients' condition.
  • Deliver high-quality patient care, including administration of medication, wound care, treatments, and comfort care.
Requirements:
  • Active and current license to practice as a professional registered nurse (RN) in the state of PA.
  • Wound, Ostomy and Continence Nurse (WOCN) certification required.
  • At least one year of nursing experience.
  • Prior home care experience strongly preferred, but not required.
  • Ability to travel to cases as assigned.
