Key Responsibilities- Collaborate with physicians, hospital case managers, and referral sources to facilitate smooth transitions.
- Conduct skilled conversations with patients, families, and healthcare providers to understand their needs and preferences.
- Maintain a deep understanding of hospital and post-acute healthcare systems, including home health, home infusion, and hospice services.
- Develop and implement transition plans that meet the unique needs of each patient, ensuring the right care at the right time.
- Provide education and support to patients and families on home care services, benefits, and eligibility requirements.
- Build and maintain strong relationships with healthcare professionals, including primary care physicians and hospital staff.
- Stay up-to-date on industry developments, including changes to Medicare and Medicaid policies, and apply this knowledge to improve patient outcomes.
- Bachelor's degree in a healthcare-related field or equivalent experience.
- 2-3 years of nursing experience, preferably in hospital or long-term care settings.
- Strong understanding of home health eligibility admission requirements, COPs, PDGM knowledge, and risk scoring/data analysis.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to work in a fast-paced environment, prioritizing multiple tasks and deadlines.
