We are seeking a skilled Social Work Specialist to join our team at PIH Health. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and support.

Key Responsibilities
  • Conduct thorough patient assessments to identify social, emotional, and economic factors that may impact their care.
  • Develop and implement individualized treatment plans to address patient needs and promote optimal health outcomes.
  • Collaborate with multidisciplinary teams to coordinate patient care and ensure seamless transitions.
  • Provide psycho-social therapy and support to patients and their families.
  • Stay up-to-date with regulatory requirements and ensure compliance with relevant policies and protocols.
Requirements
  • Master's degree in Social Work from an accredited institution.
  • Registered Associate Clinical Social Worker (ACSW) or equivalent certification.
  • Strong communication and interpersonal skills.
  • Experience in a healthcare setting, preferably in social work or a related field.
  • Familiarity with regulatory requirements related to child/elder abuse, substance use/abuse, and other legal requirements.
