Responsibilities:- Complete accurate and timely record keeping for resident admissions, transfers, and discharges.
- Coordinate with nursing staff and support personnel to plan daily services, programs, and activities that meet resident needs.
- Administer medications and review medication cards for completeness and accuracy.
- Review resident care plans and revise as necessary to ensure quality care.
- Provide leadership and guidance to nursing personnel and make daily rounds to ensure direct care is provided in accordance with resident care plans.
- Develop and maintain positive relationships with residents, family members, and other healthcare professionals.
- Current, unencumbered LPN license in the state of Oregon.
- Experience with Electronic Medical Records and computer documentation systems.
- Ability to maintain confidentiality and adhere to HIPAA guidelines.
