Role Summary
As a valued member of the interdisciplinary care team at St. Croix Hospice, the Hospice Aide plays a crucial role in delivering compassionate and professional care to patients in need. Under the supervision of a Registered Nurse, you will ensure quality, continuity, and safety in patient care while maintaining meticulous documentation.
Key Responsibilities
- Deliver essential patient care services, including bathing, personal hygiene, dressing, and maintaining a clean environment.
- Facilitate respectful and confidential communication with patients and their families.
- Monitor and document vital signs, assisting patients with self-administered medications as necessary.
- Plan and prepare nutritious meals, providing assistance with shopping and feeding when required.
- Evaluate and report any changes in the patient's condition to the RN Case Manager.
- Support patients with mobility, exercise, and therapeutic activities to enhance their quality of life.
