Role Summary

As a valued member of the interdisciplinary care team at St. Croix Hospice, the Hospice Aide plays a crucial role in delivering compassionate and professional care to patients in need. Under the supervision of a Registered Nurse, you will ensure quality, continuity, and safety in patient care while maintaining meticulous documentation.

Key Responsibilities
  • Deliver essential patient care services, including bathing, personal hygiene, dressing, and maintaining a clean environment.
  • Facilitate respectful and confidential communication with patients and their families.
  • Monitor and document vital signs, assisting patients with self-administered medications as necessary.
  • Plan and prepare nutritious meals, providing assistance with shopping and feeding when required.
  • Evaluate and report any changes in the patient's condition to the RN Case Manager.
  • Support patients with mobility, exercise, and therapeutic activities to enhance their quality of life.
