Key Responsibilities:
- Collaborate with healthcare providers and clinic personnel to identify patients at elevated risk.
- Facilitate comprehensive care for patients with chronic conditions by:
- Conducting thorough health assessments.
- Performing medication reconciliations.
- Implementing Motivational Interviewing techniques and Self-Management Goal setting.
- Providing essential patient education.
- Creating referrals to suitable agencies and resources.
- Support the transition of patients with chronic conditions from inpatient to outpatient care by:
- Assessing transitional needs.
- Conducting medication reconciliations.
- Establishing and reviewing contingency plans.
- Delivering patient education.
- Assisting with post-discharge necessities such as prescriptions, transportation, Durable Medical Equipment (DME), and follow-up appointments.
- Coordinate with healthcare providers to establish or update care plans.
- Ensure accurate and timely documentation in the electronic medical record.
- Participate in daily huddles and Patient Care Coordination meetings.
- Prepare precise reports for weekly meetings as required.
- Maintain ongoing competence in nursing practice and stay updated on current evidence-based practices.
- Perform clinical tasks within the scope of practice as needed.
- Fulfill other related duties as assigned.
Qualifications:
- Bachelor's degree in Nursing or an Associate's degree in Nursing with a minimum of 4 years of RN experience.
- Active and unrestricted RN license in the state of employment.
- Current BLS certification.
- At least 2 years of experience in a clinical setting, such as a physician's office or hospital.
- Knowledge of chronic diseases, particularly COPD/asthma, diabetes, CHF, and IHD.
- Proficient in using electronic medical records.
- Demonstrated ability to work effectively with multidisciplinary teams.
- Excellent verbal and written communication skills.
- Strong organizational and prioritization abilities.
Preferred Qualifications:
- Experience in patient education and motivational interviewing.
- Fluency in both English and Spanish.
Physical and Mental Requirements:
- Ability to lift and move objects weighing up to 35 pounds.
- Capability to sit or stand for extended periods.
- Proficient fine motor skills for operating office equipment.
- Ability to comprehend and follow verbal and written instructions.
- Logical reasoning skills for problem-solving.
