We are seeking a skilled Registered Nurse to join our team as an MDS Coordinator. In this role, you will be responsible for assessing residents' needs and creating personalized care plans based on those assessments.
Key Responsibilities:
- Manage the Resident Assessment Instrument (RAI) process to assess residents' individual needs and create unique care plans.
- Promote cross-functional communication to ensure care plans are followed.
- Ensure appropriate reimbursement for resident care services.
- Stay up-to-date with current federal, state, and local standards, guidelines, and regulations.
Requirements:
- Current, valid state RN license.
- At least 2 years of MDS experience.
- Prior experience in Long-Term Care is required.
