**Job Summary:**

The Clinical Advancement Nurse will play a critical role in providing high-quality care to patients with chronic kidney disease. This position involves working closely with healthcare professionals to develop and implement individualized treatment plans, educating patients and their families on proper care techniques, and monitoring patient progress. The ideal candidate will have excellent communication skills, a strong understanding of nephrology nursing principles, and the ability to work effectively in a fast-paced environment.

**Responsibilities:**

  • Develop and implement individualized treatment plans for patients with chronic kidney disease
  • Provide education and training to patients and their families on proper care techniques
  • Monitor patient progress and adjust treatment plans as needed
  • Collaborate with healthcare professionals to ensure seamless care coordination
  • Participate in quality improvement initiatives to enhance patient outcomes

**Requirements:**

  • Graduate of an accredited School of Nursing
  • Current appropriate state licensure
  • Minimum of 9 months experience as a Registered Nurse (preferred)
  • Chronic Home dialysis or hemodialysis experience (preferred)
  • CPR BLS Certification
