About the Role

We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team in Advanced Illness Management at Illumia. As a key member of our clinical team, you will play a vital role in delivering high-quality care to patients with advanced illnesses.

The successful candidate will have excellent leadership, interpersonal, written and verbal communication skills, as well as independent decision making and self-motivation. You will work collaboratively with the Professional Services Corporation Physician, Primary Care Physician, and other palliative team members to deliver advanced illness management and palliative care.

Key Responsibilities:

  • Diagnose, treat, and manage patients' advanced illnesses while providing emotional and social support to improve quality of life.
  • Develop individualized treatment plans in consultation with patients, families, and other members of the Advanced Illness team.
  • Facilitate obtaining care for patients when a need is identified in accordance with the Plan of Care.
  • Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.

You will also be responsible for prescribing medications, ordering laboratory, imaging, and other diagnostic tests within the scope of professional practice, and communicating clinical findings to the PSC Physician as needed.

About You

To succeed in this role, you will require:

  • A Master's degree in Nursing from an accredited school of nursing.
  • A minimum of two years Nurse Practitioner experience.
  • Nurse Practitioner specialty in Family or Adult Medicine preferred.
  • Value Based Care practice experience preferred.
  • Practice experience with Part B billing.
  • Able to work autonomously in an Advanced Practice Role.
