Youth Services Case Manager

We are seeking a dedicated Care Coordinator to join our dynamic team at Kitsap Mental Health Services. As a Care Coordinator, you will work closely with families, providers, and other stakeholders to develop and implement individualized plans that address the unique needs of each client.
Key Responsibilities Include:
* Coordinating care services for an assigned group of clients, including direct services such as individual, group, and family counseling, crisis intervention, and case management.
* Collaborating with families and providers to identify and build on existing strengths and resources to support client well-being.
* Developing and maintaining partnerships with community organizations and agencies to provide seamless service delivery and maximize client access to resources.
* Maintaining accurate records of client activity and progress, including documentation of care coordination activities, referrals, and outcomes.
Requirements:
* A bachelor's degree in psychology, social services, or a related field
* A valid driver's license in the state of Washington
* Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
