As a Stroke Coordinator RN, you will play a vital role in facilitating and coordinating the Key Elements of the Stroke Certification process, ensuring that patients receive high-quality, efficient care throughout their continuum of care.
Responsibilities:- Facilitate and coordinate the Key Elements of the Stroke Certification process, including development and review of protocols, initial and ongoing training, performance improvement, and community outreach.
- Collaborate with the Emergency Department, ED Director, EMS, hospital department directors, and staff to achieve stroke certification and improve patient outcomes.
- Develop and implement process improvement initiatives for stroke certification in collaboration with the ED Director, ED Manager, Stroke Team, and Quality Management Director.
- Perform daily concurrent and retrospective review of patients treated with stroke clinical pathways for appropriateness, response times, performance, and outcomes.
- Collect and analyze stroke program patient data to assist with documentation, clinical care, and improved outcomes.
- Track, trend, and share results with neurologists, ED physicians, radiology, laboratory, and ED to determine cause for variance.
- Participate in concurrent quality improvement programs for stroke conditions with EMS personnel by reviewing patient-level data.
- Promote education related to stroke and cardiac conditions to identified units and other hospital personnel.
- Support and collaborate with Hospital/Education and Marketing to develop community outreach/educational programs.
- 5 years' clinical experience, including telemetry.
- Familiarity with the neuroscience patient population and stroke evidenced-based medicine requirements.
- Proficient in a variety of software and computer programs, including Cerner, Outlook, Excel, and PowerPoint.
- Knowledge of process improvement/quality management methodologies, including statistical analysis.
- Proficiency in ASA, AHA, GWTG Stroke data collection, data definitions, and abstraction guidelines.
- Knowledge of regulatory compliance and experience in facilitating and coordinating the Key Elements of the Stroke Certification process.
