Key Responsibilities:
- Conduct in-home visits to assess patient health status, educational, and psychosocial needs of the patient and their family.
- Develop comprehensive individualized plans of care and targeted interventions to help patients achieve desired goals, working closely with patients, families, and healthcare providers.
- Provide education and support to patients and families on managing chronic conditions, medications, and lifestyle changes.
- Identify and address barriers to care, including social determinants of health, and connect patients with community resources and support services.
- Collaborate with patients, healthcare teams, and community physicians to ensure coordinated care.
- Continually monitor patient response to the plan of care and revise as needed.
- Maintain required documentation for all care management activities.
- Participate in case conferences and care coordination meetings to ensure holistic patient care.
Requirements:
- Bachelor's Degree in Nursing (BSN) from an accredited institution.
- Active RN license in good standing with your state medical board (AZ, TX, or IL).
- Basic Life Support (BLS) certification.
- Current driver's license, reliable transportation, car insurance, and an acceptable driving record.
- 2+ years of experience in adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting within the past five years.
Preferred Qualifications:
- Current Certified Case Manager (CCM) certification.
- 5+ years of experience in nursing.
- Comfortable working with technology and experience with innovative care teams.
Compensation: The estimated salary range for this position is $66,950 - $86,910 per year, depending on location and experience. Included Health offers a comprehensive benefits package, including remote work options, 401(k) savings plan, medical, vision, and dental coverage, and generous paid time off.
