  • Conduct in-home visits to assess patient health status, educational, and psychosocial needs of the patient and their family.
  • Develop comprehensive individualized plans of care and targeted interventions to help patients achieve desired goals, working closely with patients, families, and healthcare providers.
  • Provide education and support to patients and families on managing chronic conditions, medications, and lifestyle changes.
  • Identify and address barriers to care, including social determinants of health, and connect patients with community resources and support services.
  • Collaborate with patients, healthcare teams, and community physicians to ensure coordinated care.
  • Continually monitor patient response to the plan of care and revise as needed.
  • Maintain required documentation for all care management activities.
  • Participate in case conferences and care coordination meetings to ensure holistic patient care.

Requirements:

  • Bachelor's Degree in Nursing (BSN) from an accredited institution.
  • Active RN license in good standing with your state medical board (AZ, TX, or IL).
  • Basic Life Support (BLS) certification.
  • Current driver's license, reliable transportation, car insurance, and an acceptable driving record.
  • 2+ years of experience in adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting within the past five years.

Preferred Qualifications:

  • Current Certified Case Manager (CCM) certification.
  • 5+ years of experience in nursing.
  • Comfortable working with technology and experience with innovative care teams.

Compensation: The estimated salary range for this position is $66,950 - $86,910 per year, depending on location and experience.
