Job Summary:
The Care Coordinator RN is a Registered Nurse assigned to the Financial Clearance Department, responsible for facilitating the physician care plan for patients' hospital encounters.
Key Responsibilities:
- Collaborate with interdisciplinary teams, including physicians, case managers, and hospital leadership, to assess patient needs and develop comprehensive care plans.
- Act as a patient advocate, ensuring compliance with federal, state, and third-party utilization management regulatory requirements.
- Work closely with physician offices, physician advisors, and point-of-entry case managers to integrate clinical care and optimize patient outcomes.
- Develop and maintain strong relationships with patients, families, and healthcare providers to ensure seamless care coordination.
Requirements:
- 3 years of experience in nursing, with a focus on clinical case management or utilization management.
- Graduate nursing degree required, with a Bachelor's degree preferred.
- Registered Nurse licensure required, with Certified Case Manager or Commission for Case Management Certification preferred.
Work Schedule:
Monday to Thursday, 9:00 am to 6:00 pm, with no weekend or call requirements.
