Key Responsibilities- Coordinate patient care plans, ensuring timely and effective transitions between hospital and post-acute settings.
- Collaborate with multidisciplinary teams, including physicians, nurses, and social workers, to develop and implement comprehensive care plans.
- Conduct daily reviews of patient medical records to ensure accurate level of care determinations and facilitate timely discharges.
- Develop and maintain relationships with patients, families, and caregivers to ensure informed decision-making and optimal care outcomes.
- Stay up-to-date on regulatory requirements, payer policies, and industry best practices to ensure compliance and quality care.
- Graduate of an accredited school of nursing.
- Two years of nursing experience, with a focus on case management or care coordination.
- BSN or related Bachelor's degree preferred.
- Knowledge of healthcare financial and payor issues, as well as state, local, and federal programs.
- Use of InterQual criteria and other industry-standard tools.
