We are seeking a compassionate and skilled Home Health Hospice Care Specialist to join our team at UnityPoint Health. As a key member of our interdisciplinary team, you will provide personalized care and companionship to patients in their homes, ensuring their physical, emotional, and social needs are met.
Responsibilities- Provide personal care and homemaker services to patients, including bathing, grooming, and light housekeeping.
- Assist patients with daily living activities, such as toileting, incontinence care, and oral hygiene.
- Complete telephony care plans accurately and in a timely manner, adhering to hospice program policies and procedures.
- Establish and maintain positive relationships with patients and their families, while maintaining professional boundaries.
- Report changes in patient needs to the RN case manager, ensuring continuity of care.
- High School diploma or equivalent required; completion of a Nursing Assistant Training Program (minimum 75 hours) and valid certification in Iowa or Illinois.
- Proficiency in computer applications, including Excel, Word, and Access.
- Valid driver's license and automobile insurance (when driving for work-related purposes).
- Age-specific care and reporting of Dependent Adult and Child Abuse training within one month of employment.
- CPR certification and Mandatory Reporter course completion by state requirement.
