We are currently seeking a Social Worker-BSW to join our team. The Social Worker-BSW is responsible for conducting psychosocial evaluations and implementing suitable interventions, referrals, and counseling for patients enrolled in the designated program. This role involves collaboration within a multi-disciplinary community health team, addressing the needs of a diverse and multicultural patient base.

Key Responsibilities:

  • Conduct comprehensive psychosocial evaluations to identify clients requiring interventions; perform regular reassessments.
  • Maintain an updated directory of community emergency resources (food, clothing, shelter, counseling) for each assigned program, collaborating with community partners to assist clients in accessing these resources.
  • Facilitate psychosocial interventions, including individual, family, and group counseling sessions.
  • Provide appropriate referrals, promoting internal connections to FHCSD services such as Pediatrics and HIV testing.
  • Collaborate with the multi-disciplinary healthcare team, contributing insights on patient cases and working together to develop proactive solutions and comprehensive treatment strategies.
  • Perform additional duties as necessary.

Qualifications:

  • Bachelor's degree in Social Work is mandatory.
  • A minimum of 3 years of experience in community health services is required.
  • Ability to travel within the region as needed, with proof of liability and property damage insurance for the vehicle used. DRIVER REQUIREMENTS: Must have a valid license for at least 3 years; no more than 2 violations or accidents in the past 3 years; no vehicle-related suspensions; no DUI or reckless driving offenses in the last 7 years.
  • Equivalent combinations of education and experience that meet state or federal certification requirements may also be considered.

Skills and Experience:

  • Strong interpersonal skills to foster positive interactions with clients and colleagues within a diverse multi-disciplinary team.
  • Ability to manage multiple tasks, adapt to changing priorities, and maintain accuracy in a fast-paced environment.
  • Capacity to provide support, structure, empathy, and sound judgment in understanding human behavior and family dynamics.
  • Basic computer skills to meet departmental needs, including electronic medical record documentation and patient follow-ups.
  • Bilingual proficiency in English and Spanish is required.
  • Familiarity with behavior modification theories and current psychosocial assessment practices.
  • Knowledge of community resources and health/social service systems in the region, with the ability to build relationships with community partners.
