- Post-Discharge Calls and Patient Health Questionnaires: Follow up with patients after discharge and complete patient health questionnaires to monitor progress.
- Licensure and Education: Licensed as a Social Worker in the state of practice (preferred, required within 6 months of hire). Bachelor's degree in Social Work (required). Master's degree in Social Work or Social Administration (preferred).
- Work Experience: 1 year in social or human services setting (required).
