Responsibilities:

  • Travel to local referring hospitals to assist with discharge planning for patients needing extended, acute care.
  • Identify post-ICU patients who have entered into a state of chronic critical illness and will benefit from Select Specialty Hospital's specialized extension of acute care.
  • Collaborate with clinical teams to develop personalized plans of care that focus on mobility, weight-bearing, ambulation, and nutrition.

Qualifications:

  • Registered Nurse (RN) or Respiratory Therapist (RT) License.
  • Experience working in a hospital, critical care, or clinical liaison role.
  • Strong communication and interpersonal skills.
