Position Summary:

The MDS (Minimum Data Set) Coordinator plays a crucial role in generating statistical reports that comply with relevant legal, accrediting, and licensing standards for healthcare facilities. This position involves gathering and analyzing data from health records and personal assessments to evaluate the quality of care and conduct utilization reviews.

Key Responsibilities:

  • Plan, direct, and coordinate MDS assessments and care plan services effectively.
  • Conduct health assessments for all residents upon admission to create initial care plans and perform quarterly assessments thereafter.
  • Collaborate with healthcare providers to develop, implement, and evaluate individualized care plans for each resident.
  • Analyze and accurately code diagnoses, operations, and procedures from health records using appropriate classification systems.
  • Organize and lead quarterly care plan conferences involving residents, families, and physicians.
  • Develop and coordinate quality management, utilization review, and discharge planning in consultation with healthcare providers.
  • Review documentation each quarter to ensure compliance with established standards.
  • Meet all deadlines for reports and documentation as mandated by state and federal regulations.
  • Actively engage in the QAPI (Quality Assurance Process Improvement) program to enhance quality care and services.
  • Monitor compliance with accrediting and regulatory body criteria.
  • Participate in continuing education to maintain credentials and enhance professional knowledge.
  • Exhibit a high level of initiative and attention to detail in completing assessments.
  • Stay updated in the nursing field through participation in relevant meetings and educational programs.
  • Demonstrate a compassionate understanding of the elderly population.
  • Foster a positive atmosphere for residents in alignment with our core values.
  • Adhere to established policies and procedures to ensure quality resident care.

Qualifications:

  • Bachelor of Science degree in Nursing preferred.
  • Three to five years of nursing experience.
  • Active RN/LPN license in good standing.
  • Proficiency in clinical software, with specific MDS experience preferred.
  • Ability to communicate effectively and maintain professional relationships with residents and staff.

Job Type: Full-time
