Job Summary:We are seeking a skilled and compassionate RN Case Manager to join our team. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.
Responsibilities:- Perform assessments and procedures to evaluate patient needs and develop effective care plans.
- Develop working relationships with discharge planners and social service workers to ensure seamless transitions of care.
- Assess and review the matching of employee skills to client needs to ensure optimal care delivery.
- Visit patients in their homes to evaluate the quality of care being provided and review clinical notes.
- Participate in ongoing interdisciplinary assessment of the patient to ensure comprehensive care.
- Graduate of an accredited school of nursing with a current license as a Registered Nurse in the State of MN.
- Able to travel within a geographic area serviced by the office, with reliable transportation, a valid driver's license, and proof of auto insurance.
- CPR Certification is required.
- Two (2) years of current nursing experience, with one (1) year of case management experience preferred.
- Community health and a wide range of Medical-Surgical experience are preferred.
- Annual Salary Range: $89,905 - $106,233.
- $6,000 Hiring Bonus.
- Mileage Reimbursement / Company Car Program.

- 100% Clinical License Reimbursement.
