Responsibilities

  • Manage clinical care and rounding for patients at assigned skilled nursing facilities (SNFs).
  • Conduct daily rounds and prescribe necessary medications and treatments.
  • Deliver high-quality, patient-centered primary care services to residents.
  • Ensure timely documentation of all patient interactions.
  • Be available for consultations via phone or secure messaging with nursing staff.
  • Utilize Theoria's proprietary electronic health record (EHR) system, ChartEasy, for documentation.
  • Familiarity with commonly used EHR systems such as PointClickCare, MatrixCare, and American HealthTech is beneficial.
  • Maintain punctuality and reliable attendance for scheduled shifts.
  • Possess reliable transportation for commuting.

Shift Structure

  • Rounding occurs from 7 AM to 1 PM local time, requiring availability for nursing staff and addressing urgent issues.
  • Shifts typically commence around 7 AM local time, with reasonable flexibility.
  • Workdays are Monday through Friday.
  • Charting and documentation can be performed off-site.
  • This position requires in-person attendance.
