Centralized Intake Coordinator

Under the direct supervision of the Communications Coordinator, the specialist's responsibilities include conducting screenings of potential new clients and making internal and external referrals for these new clients to Astor or outside providers. The specialist will continue to engage with these families post-linkage to ensure that connections and intakes have been successful, as well as outreach to recently admitted clients across agency programs to assess for additional needs.

Key Responsibilities:

• Conduct screenings of potential new clients
• Make internal and external referrals for new clients
• Engage with families post-linkage to ensure successful connections and intakes
• Outreach to recently admitted clients across agency programs

Requirements:

• B.A. or High School/Ged with appropriate experience
• 2 years experience in case management, intake assessment, or family/peer advocacy
