Role Summary:This role plays a crucial part in developing and coordinating holistic care management activities to ensure high-quality, cost-effective healthcare outcomes for our 28 million members.
Key Responsibilities:- Evaluate member needs and resources to develop effective plans for optimal outcomes.
- Create long-term care plans and identify necessary providers and resources.
- Collaborate with members, families/caregivers, and care provider teams to ensure accessible services.
- Provide support to members and their families/caregivers for various needs.
- Monitor care plans, member status, and outcomes, providing recommendations for improvement.
- Interact with healthcare providers and partners to ensure member needs are met.
- Gather, document, and maintain member information and care management activities.
- Conduct home visits to assess member needs and collaborate with healthcare providers.
- Offer education to members and their families/caregivers on procedures, service options, and healthcare benefits.
- Bachelor's degree and 2-4 years of related experience or equivalent experience acquired through accomplishments.
- RN or LCSW required; two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience.
$43.58 - $76.36 per hour, plus a comprehensive benefits package including competitive pay, health insurance, 401K, tuition reimbursement, paid time off, and flexible work arrangements.
