Job Summary

The Social Worker plays a vital role in the coordination of care for mental health, behavioral, and social needs. This position requires collaboration with the team to facilitate communication between patients, families, healthcare professionals, community, and state agencies, as well as other interdisciplinary team members.

Key Responsibilities
  • Assist with the implementation of care plans and coordination of behavioral health and social services.
  • Function as a member of a professional team providing evaluation and treatment for psychiatric patients, including psychological assessment, crisis intervention, case management, patient advocacy, care coordination, and community outreach.
  • Provide input in treatment decisions and present specific treatment plans for approval.
  • Obtain information from patients and other appropriate sources to initiate treatment recommendations and record all information in medical records.
  • Involve patients, families, and others as appropriate in the delivery of care and treatment recommendations.
  • Work with patients to determine goals to be achieved during the course of treatment.
  • Makes arrangements for the safe and expedient transfer of patients as required.
  • Acts as a resource of information for Division members, patients, referral sources, etc.
  • Assists with provider schedule maintenance and sets up appointments, arranges for follow-up treatment as indicated.
  • Strives to support quality patient care, patient satisfaction, and assist in the accomplishment of related goals.
  • Assists with insurance verification and authorization procedures.
  • Part of an interdisciplinary team ensuring patients progress in the acute episode of care through post-discharge and is quality-driven while being efficient and cost-effective.
  • Interacts with patients, family members, healthcare professionals, and community and state agencies.
Additional Responsibilities
  • Serve as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information.
  • Identify high-risk patients from a psychosocial/financial perspective, assess their psychological needs, and provide information, support, counseling, care management, and referrals to appropriate resources.
  • Resolve psychosocial patient care issues and develop and implement a complex patient transition/discharge plan.
Requirements
  • Bachelor's Degree in a related field of study (required), Bachelor's Degree in Social Work (preferred).
  • Certification(s) and License(s) as required by the state and employer.
