Job Overview

Position Summary: The Youth Support Specialist fosters constructive relationships with young individuals (ages 5 – 21) and their families facing significant behavioral and/or physical health challenges, aiming to assist them in achieving enhanced overall health and social functioning.

Key Responsibilities:

Exhibit understanding of childhood psychiatric conditions, chronic health issues, and complex trauma. Engage in outreach initiatives through diverse methods to connect with individuals experiencing chronic health conditions, mental health challenges, and/or substance use disorders, often concurrently. Perform initial and ongoing comprehensive evaluations (CANS-NY, Complex Trauma Assessment, Comprehensive Assessment, etc.) to identify strengths and needs. Develop and update care plans that reflect the needs and personal objectives of members, focusing on health and wellness maintenance. Maintain regular contact with members at least monthly, providing telephonic and/or in-person outreach, engagement, and comprehensive service planning in the community. Advocate for members to ensure access to essential resources that promote wellness/self-management and reduce the frequency of emergency room visits and hospital admissions. Oversee and coordinate all care for members, including access to and maintenance of medical insurance, linkage to treatment providers, and community resources. Collaborate with community providers as part of a multidisciplinary team to ensure goal-oriented care planning. Conduct crisis intervention as necessary and follow up appropriately. Maintain accurate, timely, and detailed record-keeping in an electronic medical record system. Coordinate with the Program Coordinator/Supervisor and health home outreach team promptly to ensure accurate caseload status (including enrollments, closures, and screen outs). Complete all required monthly documentation to ensure continuity of engaged members' medical insurance and accurate billing. Work collaboratively within a care management team, attending and contributing to team meetings to provide insights and share resource information regarding member needs, issues, and concerns. Manage daily office and field schedules with other team members and the Program Coordinator/Supervisor, as well as document schedules for the office manager. Provide resources and serve as a consultant to all team members on medical, psychosocial, substance use issues, and social service needs. Deliver HHSC YCM services to youth across the designated service area as assigned. Participate in ongoing training to enhance skills and meet DOH/HHSC training requirements. Engage in regular individual supervision to address concerns and enhance skill development. Participate in weekly High Risk Review meetings (as appropriate) to monitor member concerns and high-risk behaviors, linkages to supports, and member/family needs. Attend monthly Youth Care Management meetings to discuss agency policy updates, HHCM policy updates, paperwork, billing, training, resources, etc. Be responsible for agency vehicles, including maintenance, documentation, and fuel card when assigned. Manage agency cell phone, laptop, and related items. Adhere to program guidelines as outlined in the personnel manual. Report any issues or concerns to program administration regularly or as needed while working in the community. Ensure access to an insured personal vehicle. Track and submit mileage. Perform other duties as assigned.

Qualifications

A Bachelor’s degree in Arts or Science with two years of relevant experience (care coordination, youth, and families), or a Registered Nurse license with two years of relevant experience, or a Master’s degree with one year of experience. *Must maintain CANS-NY certification. *Must possess a valid driver's license and car insurance.
